Benefits of Membership
Thank you for thinking of becoming a member.  It costs £10 per household per year.

All patients of Davenport House Surgery are eligible to become members of the Patient Participation Group. For reasons of patient confidentiality, we have no access to names or addresses of patients, so we have to ask you to contact us initially.

As a member you can:

Enjoy reading the lively magazine delivered to your home four times per year, keeping you up to date with developments at the surgery and on wider medical issues.

Have free access to the presentations and ‘members only’ small education groups on health topics that interest you.

Be part of the dialogue between patients, doctors and other medical staff on practice related matters, and have your say on local health issues.

Meet the doctors at the Annual General Meeting of the Patient Group.

Contribute to and benefit from such Patient Group achievements as the touch-screen booking in system and the lay-by for disabled drivers at the Surgery.

Alternatively the form (in word format or scanned version of the pdf version) can be sent by email to membership@patientgroup.org.uk

It helps us to keep costs down if you elect to pay by Standing Order, (one payment per family of £10 in March each year), but we are happy to accept payment by cheque, cash or PayPal if you prefer.

Our PayPal address is website@patientgroup.org.uk. When using PayPal please select your physical address as the delivery address so that we can easily identify your payment.

You can set up an annual Paypal payment (£10.50 per family per annum) by clicking on the PayPal icon on the right of the page.

If you cannot print the application form copies are available at the surgery.

Alternatively, you can obtain a copy by sending an email to the Membership Secretary (membership@patientgroup.org.uk) or dropping a note to the Membership Secretary at the above address, giving your name and address, and we will post a form to you.
